Review Checklist

It is important to bring specific items along on your visit.

• Your insurance card
• Glasses or contact lenses that you currently use or are prescribed to you
• List of prescription and other medicine that you currently use (with dosages)
• List of prescription and other medicine that you currently use (with dosages)
• The social security # of the individual listed on the insurance card
• Payment for your insurance co-pays, deductibles amounts, or other fees determined by your insurance coverage

We ONLY bill medical insurance. We DO NOT bill vision insurance.
