Be prepared for your first visit by looking over the checklist below. Download the office forms listed to the right and fill them out before your visit to save time!
- Your insurance card
- Glasses or contact lenses that you currently use or are prescribed to you
- List of prescription and other medicine that you currently use (with dosages)
- The social security # of the individual listed on the insurance card
- Payment for your insurance co-pays, deductibles amounts, or other fees determined by your insurance coverage
We ONLY bill medical insurance. We DO NOT bill vision insurance.
